Are You Living With Illness?

We understand that living with a serious illness often leads to many questions and some difficult decisions. You may have heard of hospice, but may not be sure if you are ready for hospice care in your current situation. At Hospice of Helping Hands, we want you to know that we can help you and your family through this difficult time.

Below is a list of some of the primary services that hospice care can provide. Please keep in mind that these lists are not all inclusive, and you may choose not to receive all hospice services. Your hopes, your priorities, your comfort and your quality of life define our care.

What Hospice of Helping Hands Can Do For You:

  • Control pain and symptoms you are experiencing
  • Help with advance-care planning
  • Assist in coordinating services that are independent of our organization
    (Example, Meals on Wheels)
  • Provide a spiritual advisor
  • Arrange for a volunteer companion
  • Provide counseling or a comfortable atmosphere to voice concerns, hope or reminisce
  • Assistance with personal activities of daily living
    (Example, bathing)
  • Dietary assistance
  • Relieve the financial burden of all medications and supplies related to the hospice diagnosis
  • 24/7 availability
  • Care is provided in the home - wherever home may be (Including assisted living facilities and nursing homes)

What Hospice of Helping Hands Can Do For Your Family:

  • Arrange for respite care
  • Provide counseling or a comfortable atmosphere to voice concerns, hope or reminisce
  • Provide bereavement services, including counseling and support groups
  • And of course, all the benefits of hospice care for yourself are benefits to your family as well - they only want you to have the highest quality of life possible for you

Informal Hospice Eligibility Questionnaire

You may be eligible for hospice care if you check four or more statements. However, your primary physician and our Medical Director will make the final decision regarding eligibility. Please check the following statements as they apply to you or your loved one to see if hospice care may be appropriate.

I have started feeling more tired and weak.
I experience shortness of breath, even when resting.
I spend most of the day in bed or in a chair.
I have noticed an increased weight loss in the past six months.
I make frequent phone calls to my physician.
I take medications to lessen physical pain.
I have fallen several times in the past six months.
I have made frequent trips to the emergency room in the past six months.
I need help from others with important daily activities. (bathing, dressing, eating, cooking, walking, getting out of bed)

I have had to take an antibiotic for infection 3 or more times in the past six months.

My physician has told me my life expectancy is limited or terminal (call now)
